Your A1C Looks Fine. Your Kidneys May Not Be.

If you are diabetic, your kidneys are failing before your blood sugar ever tells you. And your doctor is only watching the blood sugar. I need to say that plainly, because in 22 years as a kidney specialist — 20 of them in Zurich — it is the most expensive misunderstanding I see in American patients. You have been trained to watch one number. Your A1C. Your glucose. Your morning finger-prick. And you have done it well. You count the carbs. You take the metformin, the insulin, maybe a blood pressure pill too. You watch the line on the meter. But nobody told you that while you were watching your blood sugar, the sugar was quietly destroying the one organ that never gets mentioned at the appointment. Diabetes is the number one cause of kidney failure on earth. Not high blood pressure. Not age. Diabetes. Roughly one in three diabetics will develop kidney disease — and most do not find out until the filtration has been declining for years. By then the conversation is no longer about prevention. It is about dialysis. Here is why it stays hidden for so long. ===== Your kidneys contain microscopic filtration units called glomeruli. Think of them as the finest mesh net your body produces — so fine that a human hair looks like a rope compared to the threads it is made of. Now picture pushing syrup through that net. Every hour. Every day. Year after year. That is what elevated blood glucose does. The sugar in your blood is not just a number on a meter — it is a physical, abrasive load on those filtration threads. And it drives three specific mechanisms that destroy them. The first is the TLR4/NF-kB inflammatory cascade. High glucose feeds it directly. It frays the glomerular threads one by one — while your blood sugar reading looks "controlled." The second is mitochondrial oxidative stress. Your kidneys filter 200 litres of blood every single day. In a diabetic, that blood carries a constant oxidative load, and the cells doing the filtration work accumulate damage faster than in anyone else. This is why diabetic kidneys age decades ahead of schedule. The third is fibrosis — scar tissue building permanently inside the kidney, replacing filtration capacity that does not come back. A 2025 paper in Frontiers in Pharmacology confirmed the specific anti-fibrotic pathways involved. In diabetics they are almost always left dormant. Three mechanisms. Running at the same time. Driven by the very sugar you have been told to manage. ===== And here is what makes me angry. Your blood sugar medication manages the sugar. It does not touch the cascade. It does not protect the mitochondria. It does not activate the anti-fibrotic pathways. Metformin, insulin, a GLP-1, a blood pressure pill — they lower the glucose load and the pressure. That matters. But lowering the glucose does not undo the inflammation, the oxidative damage, or the fibrosis already running underneath. It is managing the pressure of a flood while the dam keeps cracking. This is why I have watched diabetic patients with beautifully controlled A1C still lose kidney function year after year. The number on the meter looked fine. The filtration was failing anyway. And the cruelest part: the first real sign is invisible to you. Not pain. Not swelling. Just a tiny amount of protein leaking through the damaged net — measured by a test called the UACR. It shows up years before your eGFR drops. Years before "stage 3" ever appears on a chart. Most diabetics have never once had it explained to them. ===== There is published research on what actually reaches those three mechanisms. And I understand if none of this is familiar — most people, including most doctors here, have never come across it. It started with a puzzle. For years, researchers could not explain one particular population: the herding communities of the Tibetan plateau. They live at 3,500 metres — extreme cold, thin oxygen, hard physical lives, every condition that should destroy kidney function faster than anywhere on earth. Yet when their filtration was measured in their seventies and eighties, it looked like the kidneys of people twenty years younger. The researchers spent years trying to explain it. They ruled out genetics. They ruled out altitude. They ruled out diet and activity. In the end only one thing was left — something these communities had eaten every single morning for fifteen centuries. A fungus. Specifically, the fruiting body of Cordyceps militaris. When the laboratories studied it, they found why it mattered. Its active compound — cordycepin — does three things at once: it calms the TLR4/NF-kB inflammatory cascade, it shields the kidney-cell mitochondria from oxidative damage, and it switches on the anti-fibrotic pathways that keep filtration tissue from scarring over. The same three mechanisms high blood sugar drives. Quietly addressed, every morning, by people who had eaten it for breakfast for fifteen hundred years before anyone understood how it worked. Then modern medicine tested it directly. A randomised controlled trial — 98 CKD patients at Stage 3 and 4, many of them diabetic. Three months. eGFR rose from 31.8 to 45.6. Creatinine fell 18.3%. Urinary protein dropped 36.7%. A meta-analysis of fifteen studies and 1,310 patients found the same. So did the Cochrane review, and a 2025 paper in Frontiers in Pharmacology on the anti-fibrotic pathways. The formula I now recommend to my own diabetic patients, Adoria, is built on that exact extract — Cordyceps militaris fruiting body at 3,000mg, the dose the trial used — with astaxanthin and alpha-lipoic acid for the oxidative load diabetes creates. One scoop into the morning coffee. ===== Here is the part I cannot say loudly enough: this damage is silent. No pain to warn you. No symptom to catch it. Which means every month it goes unaddressed, the three mechanisms keep running and the filtration you have keeps slipping away — quietly, without permission. And the earlier you act, the more responsive your kidneys still are. Waiting is the one thing that makes it harder to turn around. So I put together everything I tell my diabetic patients — the one test to ask your doctor for instead of just your A1C, what the research actually shows, and exactly how I have them address all three mechanisms at home. If you are diabetic, this is the conversation no one ever had with you. It's worth ten minutes today. 👉 Read what Dr. Galvin tells her diabetic patients: https://adoriabrand.com/pages/diabetes — Dr. Andrea Galvin, Kidney Specialist, 22 Years

What dermatologists NEVER tell you!

As a dermatologist I treated over a 1000 teenagers with acne. Last week was my last day at work. Retirement. I'm going to share what I wasn't allowed to say for the last 28 years. Acne during puberty always has a cause. And most treatments don't actually treat that cause. Although, I prescribed them anyway. What else is a doctor supposed to do? The NHS pays for it, parents hope for the results and you do what you were trained to do. I've seen mothers come back months after treatment. Their child still had inflamed spots and felt terrible about it. Didn't even want to go to school sometimes. And I just wrote them another prescription. I knew all of this and kept going anyway. Because that was my job. Until my own daughter Emma sat crying in the kitchen at 14. I could see it on her face before she said a word. Inflamed acne with redness everywhere. She also said she didn’t want to go to school because of it. That destroyed me. Not because I wasn't a doctor. But because as a doctor I knew exactly what I would prescribe her and what it would do to her. Let me tell you what I know, and what I stayed silent about for years. During puberty something happens in the child's skin. Hormone levels rise, the oil glands become much more active and produce more oil than the skin can handle. This oil mixes with dead skin cells and clogs the pores. Then comes the real problem, a bacterium called Cutibacterium acnes lives on our skin. And it loves this trapped oil. It starts growing faster. That bacterium is what causes acne. The immune system fights back, and that creates the redness, swelling and inflammation. That is the actual cause. Now comes the part I don't enjoy telling you. Most of the creams, gels and pills I prescribed are too harsh. They do kill the bacteria, yes, but in doing so they also destroy the skin's natural protective barrier. And all the good bacteria that is supposed to keep the skin healthy. And here is the problem. Teenage skin is not like adult skin. It's still developing. It's much more sensitive. When you ‘attack’ this young skin with harsh chemicals, it becomes even more sensitive. So the skin barrier gets damaged and the immune system becomes more reactive. The inflammation comes back, only worse each time. Because the skin is now even more vulnerable than before. I saw this with my own eyes. Children sitting back in my room after 3 months of treatment, with skin looking worse than when they started. This is not a coincidence. We just would never say it out loud, because it would make patients question their doctors. And that is exactly what I didn't want for Emma. So I did the only thing I could do as a mother. I started looking for an actual solution. What can really heal my daughter's skin? Not short term but properly. What targets the bacteria without destroying her skin? I came across something I overlooked for years. Calendula based products are not some exotic miracle. Not expensive technology. Calendula is a plant-based ingredient that does something no harsh gel can do. It specifically kills the bacteria that cause acne, without damaging the skin. It doesn't destroy the protective barrier. It doesn't kill the good bacteria. It only targets what is causing the problem. And at the same time this plant has an anti-inflammatory effect. It calms the immune system so redness and swelling are reduced. The skin can finally recover. And because the skin barrier stays healthy, the inflammation doesn't come back. Emma tried the balm made with a high concentration of calendula. Already in the first week I noticed a difference. The redness was visibly reduced. And for the first time in months no new pimples appeared. Not just fewer, actually none. In the 2nd week the change was clear. The inflammation that used to dominate her whole face now became much calmer. Her skin looked overall better, more settled. Emma stood in front of the mirror in the morning and I watched her enjoying it. In the 3rd week she came into the kitchen in the morning without me checking her skin first. I just smiled at her and she smiled back. After a month, she asked me if she could go out with her friend. For the first time in months. And at some point she realised that it's getting better. When she said that, it summed up everything I’ve been doing wrong for so long. And I was glad, because as a doctor I know that was only the beginning of the healing. If your child also has acne and you watch them stop socialising. If you've already tried pharmacy stuff, and the GP or even a dermatologist just keeps writing you another prescription without really listening. If you're scared of Roaccutane, but also know that things can't carry on the same way. Then read what I found. Harsh chemicals will never improve your child's skin. And I'm now officially allowed to say that. https://enduorin.com/pages/10-reasons
